faq entry describing how to create these buttons has been updated.
Are you using the latest version of Ultra Recall (version 1.4b)? Some fixes were introduced in the last version to address the focus issue you mention. You can verify the installed version in Ultra Recall at
Help | About | Install Info... :: the UltraRecall.exe version should be 1.4.2.1 for the most current version. Simply re-download and re-install the latest version install file to upgrade if you have a prior version installed.
